Nice little quiz, although I have to protest the “correct” answer to the question on grip size. Per Tom Wishon’s blog in about.com, a larger grip can prevent a player from releasing the wrists completely, causing the club to be slightly open at impact, resulting in a slice.
The grip question gets a lot of people. We did an experiment in our clubfitting schools when we offered them in the past. We had several assembled clubs that were identical except for the grip sizing. We had a leather strap wrapped onto a steel shaft without an underlisting so it was extremely small all the way to jumbo grips. We had the students hit them at the range and watch the results. Many came away disappointed that they could not see really any difference in the ball flight – but again that was the point of having the students hit them to see for themselves.
it has been my experience in fitting clubs, that fitting oversize grips prevents overcocking on the wrist which results in a straighter shot but less distance because you don’t get full wrist snap into the shot

Great quiz. Scored 9 out of 10.
But I disagree with your answer for Q5 — maybe question is somewhat misleading.
